Symptoms and treatment of cholestatic hepatosis
Cholestatic hepatosis is characterized by cholestasis, the accumulation of bile pigment in the liver cells, a violation of the trophism of hepatocytes with the development of dystrophy associated with an insufficient intake of protein.
The cause of this type of hepatosis can be a harmful effect on the liver of toxins or allergens, which are often drugs. Metabolic processes in hepatocytes in cholestatic hepatosis are impaired, mainly the exchange of cholesterol and bile acids suffers, the formation of bile and its outflow are impaired.
Symptoms of cholestatic hepatosis
With cholestatic hepatosis, itchy skin appears, jaundice develops, feces become colorless, urine turns dark, and body temperature may rise. The liver is most often enlarged. Patients feel pain and heaviness in the right hypochondrium.
When carrying out laboratory tests in the patient's blood, a high level of bilirubin and cholesterol is found, and the ESR indicator is above normal. There is a high activity in the serum of alkaline phosphatase, as well as leucine aminopeptidase. A decrease in the concentration of stercobilin leads to discoloration of feces.
A puncture biopsy may be done when a diagnosis is made. Cholestatic hepatosis causes accumulation of bile pigment in hepatocytes, reticuloendotheliocytes and in the intralobular bile ducts. Dystrophic changes in liver cells, usually of a protein nature, are also noticeable; in severe cases, cholangitis may be noted.
The disease can be acute or chronic. Prolonged cholestatic hepatosis quickly turns into hepatitis due to the occurrence of a reaction of the reticulohistiocytic stroma of the liver and concomitant cholangitis.
Treatment of cholestatic hepatosis
Treatment of cholestatic hepatosis practically does not differ from the treatment of chronic fatty hepatosis. It can be supplemented with corticosteroids. Spa treatment is not recommended for patients.
Prevention measures include the elimination of the causes that provoke the development of pathology. It is important to limit the use of alcoholic beverages, to take medications that have a hepatotropic effect, only on the recommendation and under the supervision of a doctor. It is also necessary to provide a healthy diet, timely treat diseases of the gastrointestinal tract, identify and treat diseases that can cause fatty hepatosis. Patients with fatty hepatosis require dispensary observation and regular examination by a specialist. Course treatment of the disease is carried out 1–2 times a year and more often.
