Phimosis surgery (circumcision)
Phimosis is a disease in which it becomes difficult to expose the glans penis. This not only makes it difficult to carry out hygiene procedures and causes painful sensations (for example, during an erection), but also very dangerous for men's health. After some time, with progressive phimosis, patients begin to have problems with urination: due to the narrowing of the opening of the foreskin, urine cannot be freely excreted. It comes out in a thin trickle or drops.
Phimosis is also dangerous for its complications, among which are balanoposthitis, paraphimosis and accretion of the foreskin to the glans penis. In such cases, patients are shown an operation to circumcise the foreskin (circumcision).
In circumcision, the foreskin of the penis is excised. The foreskin is a fold of skin that covers the glans of the penis. It is formed by the inner and outer surfaces (leaves of the foreskin). On the underside of the head is the frenulum of the penis, which starts from the external opening of the urethra and looks like a longitudinal narrow fold. The preputial sac is the space that is limited by the foreskin.
Indications for the operation and its performance
What kind of symptoms are the basis for the operation? First of all, this is non-removal of the glans penis from under the foreskin or difficulty in exposing it. Frequent exacerbations of balanoposthitis and scars that form on the foreskin are also dangerous: the scar tissue is less elastic, which creates the risk of cracks, ruptures and bleeding during sexual activity. Also, excision of the foreskin is carried out for hygienic and religious reasons.
Before the operation, patients undergo an examination and pass a general blood and urine test. Blood biochemistry, a blood test for AIDS, syphilis, hepatitis (B and C) and a coagulogram are also mandatory.
During the operation, the patient is under local anesthesia. General anesthesia is used much less frequently. After marking the zone of resection of the foreskin, the narrowing ring is removed. The foreskin is removed from the penis, after which the edges of the skin are sutured. It is important to correctly match the edges and aesthetically suture so that the man does not have a scar after the operation. When the procedure for suturing and forming the frenum of the penis is completed, the wound is treated with antiseptic preparations and a dry pressure bandage is applied on top. The duration of the circumcision operation is from 25 to 45 minutes.
Postoperative period
The postoperative period is characterized by some discomfort and pain at the circumcision site, but special pain relief is usually not given. It is enough to make dressings every other day or two. It is very important that the dressings are not contaminated with urine, because urine getting on the wound can cause an inflammatory process and healing will take longer. The stitches are usually removed after a week and a half. A man can resume sex life two weeks after circumcision surgery.
The operation of circumcision is extremely simple from a technical point of view, so the risk of any complications (infection, bleeding, etc.) is extremely small.
The only consequence of the operation is a decrease in the sensitivity of the glans penis, but this is often the desired effect, increasing the duration of intercourse and reducing the likelihood of premature ejaculation. Many men who have such a problem normalize their sex life precisely because of circumcision.
