As for carbamazepine, its maximum daily dose is 1200 mg, and the minimum is 600 mg. It is necessary to take the drug up to 3 times in 24 hours.

The dosage regimen for epilepsy treatment depends on the patient's condition and the form of the disease:

  • It is most convenient to use drugs that have a long-term effect. It is enough to take them 1 or 2 times a day. For example, these are Finlepsin Retard, Tegretol, Depakin Chrono;
  • When a patient suffers from generalized seizures, both carbamazepine and valproate may be prescribed. If the patient is suffering from an idiopathic form of the disease, only valproate is prescribed;
  • If the patient loses consciousness (absence) during an attack, Etosuximide is chosen for treatment;
  • New drugs, such as Tiagabin and Lamotrigine, have a proven effect. Therefore, in recent years, they are prescribed to patients more often.

If seizures have not been observed for five years, this is the basis for stopping treatment. The dosage is gradually reduced, bringing it to a minimum, and then the drug is abandoned altogether.

Removal of status epilepticus

If the patient has status epilepticus, he is removed with the help of Diazepam and Seduxen. Sibazon is indicated for intravenous slow administration. After 15 minutes, it can be entered again. If there is no effect, then the drug is replaced with Phenytoin, Hexenal or Thiopental sodium. In order not to suppress the respiratory function, after the injection of 10 ml of the drug, a minute break is required.

When intravenous drugs do not help relieve status epilepticus, inhalation anesthesia is used. For this, nitrous oxide and oxygen are used in a 2: 1 ratio. However, a contraindication to this method of treatment is a coma of the patient, impaired respiratory function and collapse.

If the patient suffers from focal epilepsy, which was provoked by tumors, aneurysms or other pathological changes in the structure of the brain, then surgery is indicated.

Most often, the operation is performed without general anesthesia, so that it is possible to monitor the patient's condition and not damage the most important parts of the brain that are responsible for thinking, movement and speech. Local anesthesia is used to relieve pain.

The operation is indicated for temporal lobe epilepsy. In this case, the temporal lobe is either subjected to resection, or the hippocampus with the amygdala is selectively removed, which is much preferable for the patient. The efficiency of surgical intervention is high, up to 90% of cases are successful, and seizures in patients no longer occur.

If children have underdevelopment of the cerebral hemisphere or hemiplegia, then the hemisphere can be removed completely. This operation is called hemispherectomy.

Callosotomy is indicated if the patient is diagnosed with primary idiopathic epilepsy. The operation is aimed at the intersection of the corpus callosum, which allows you to break the connections between the hemispheres of the brain. This, in turn, will relieve the patient from seizures.
